Getting woken up by charlie horses in my calves 3-4 nights a week now 😫 Been on semaglutide 1.0 mg for 2 months. Also getting cramps in my hands when I grip things and random twitches in my thighs during the day.

Is this an electrolyte thing? I'm admittedly not eating much — maybe 1300 cal/day. Never had muscle cramps before starting this medication.

Muscle cramps are not a commonly listed side effect of semaglutide per se, but they are a very common consequence of reduced intake on GLP-1 RAs. The culprit is almost always electrolyte imbalance.

When you're eating significantly less plus potentially losing water weight, you can become deficient in:

My recommendation: get a basic metabolic panel (BMP) and a magnesium level drawn. In the meantime, it's reasonable to supplement:

  • Magnesium glycinate 400 mg at bedtime (best absorbed, least GI upset)
  • Ensure adequate potassium through diet (avocado, banana, potato) or consider a potassium-containing electrolyte drink
  • Hydrate adequately — dehydration concentrates your electrolytes but also worsens cramps paradoxically

The magnesium glycinate at bedtime fixed my nighttime leg cramps within a WEEK. I'm not exaggerating. Went from 3-4 cramps a week to zero. It also helps with sleep which is a nice bonus.

I also want to plug LMNT electrolyte packets — they have a good ratio of sodium, potassium, and magnesium without sugar. I drink one daily and my cramps, energy, and overall hydration are all better.

One thing to be careful of: don't megadose potassium supplements. Too much potassium can cause dangerous heart rhythms. Get it from food or moderate supplementation and let your labs guide dosing.

e thing happened to me and it was specifically magnesium. My level came back at 1.5 mg/dL (low end of normal is 1.7). Doctor started me on magnesium oxide 400 mg — and I have to say, magnesium glycinate or citrate is better absorbed. I switched to glycinate on this forum's advice and it worked much better.

Also want to note: if you're having frequent diarrhea as a side effect, that depletes magnesium and potassium even faster. It's a compounding problem.

omg is THAT why my eye has been twitching for 3 weeks?? i thought i was just stressed 😂 i'm on tirz 5 mg and barely eating 1200 cal. adding this to my shopping list immediately
